Supporting documents

We need supporting documents from you to see if you can receive special assistance. Make sure to fill in the supporting documents completely. The sooner you send us your information, the sooner we can review your application.

Important before and during application

  • It is important that you first look up your supporting documents and save them on your computer. Afterwards, you can then begin with your online application. This way you will avoid the application process to be shutdown because finding all the necessary information took too long.
  • With the KopieID-app you can use your smartphone to make a safe copy of your identification document (ID). You can also use the app to make digital copies of other supporting documents. Then you can immediately send them online with your application.
  • The more completed your application, the faster we can determine your right to benefits. However, do not wait until you have all supporting documents before you send your application. You can always send them later.

Basic supporting documents

Personal data applicant and partner (if there is one)

  • Valid proof of identity (passport or ID, not a driving license)
  • If applicable: foreign nationals documents (front and back)

Bank statements or PDF printouts from online banking

These statements have to be complete, otherwise we cannot process your application.

  • Of all banking and savings accounts that you and (if there is one) your partner and children younger than 18 years.
  • Are(/were) you a self-employed entrepreneur? Please also submit statements from your business account.
  • The printouts or statements cover a minimal period of 3 months before the date of your application. This also counts for all banking and savings accounts that you have. Even if they have not been used often.

The following should be clearly visible on all printouts and statements

  • The amount of the debit
  • The date of the debit
  • Credits
  • The balance of the account
  • Date and serial number of the bank statement

Censor

Are you uncomfortable sharing all of your information? Then you have the right to censor some data. The data you are allowed to censor are:

  • Description / subject
  • The account number of the person or organisation to whom payment was made
  • The name of the person or organisation

Exception

In some cases we may still ask you to show us everything in your debits. This may happen in the following situations:

  • When you submit an application for special assistance for certain costs that you can demonstrate by means of your direct debit.
  • When there is suspicion of fraud.

Additional supporting documents

You are a tenant

  • Rental agreement

You are (co-)owner of a building

  • Deed of sale
  • Mortgage deed
  • Copy of provisional assessment from the Tax Administration for the refund on your own home (front and back)
  • The most recent overview you have of your mortgage debt and monthly mortgage payments

You are staying in an institution

  • Rental and care agreement
  • Personal contribution CAK

You have other assets (such as a boat, caravan, jewellery, paintings, shares, investments, stocks, life insurance, foreign real estate)

  • Supporting documents of these assets
  • Evidence of share in an undivided estate (such as in the event of divorce) or inheritance

Housing / living situation

  • Cohabitation agreement (if applicable)

You or your children have income from wages, other benefits or student finance

  • Proof of income with specification of the last 3 months

You have (the right to) maintenance

  • Proof of the amount and duration of the maintenance to be received, for you and your children (such as the court ruling and agreement)

You have a maintenance obligation

  • Proof of the amount and duration of the maintenance to be paid (such as court ruling and agreement)

You have your own company or work as a freelancer

  • Profit and loss statement
  • Statements from the business bank account(s) for the last 3 months
  • Deregistration from the Chamber of Commerce

You have debt(s)

  • Proof of your debts with repayment obligation
  • Decision by the subdistrict court on administration and/or WSNP (Debt Rescheduling Natural Persons Act)
